Becoming a Patient

Eligibility

To become a patient of the VIM-Jax Clinic the following criteria must be met:

Residency

You must live or work in Duval County.

Income

You must be working at least 20 hours per week and must have an income level between 150-250% of federal poverty guidelines (approximately $13,000 minimum per year).

Uninsured

You must be without private or government funded medical insurance.

After listening to the recorded message and you feel you qualify for our services then call us again to make an appointment with one of our Intake Counselors. He/She will help you fill out the application, copy your photo ID and check stubs and take an initial medical history. If you qualify for our services you will then be given an appointment for your first exam as soon as possible.

Our Hours: Tuesday – Friday 9:00am to 5:00pm
Saturday 9:00am to 12:00noon
