Become a Patient
To qualify to become a patient at our Downtown or West Jacksonville Clinic, please see instructions below. Please be sure to include all requested information and documentation. Our Downtown Clinic is located at 41 East Duval St., Jacksonville, FL 32202. Our West Jacksonville Clinic is located at 5927 Old Timuquana Rd., Jacksonville, FL 32210.
If you are unable to send in the requested qualifying information and documentation via email, you may also call 904-399-2766, press 8, and leave your full name, the clinic to which you would like to be referred (Downtown or West Jacksonville) and daytime phone number for a call back. Please speak slowly and clearly when leaving a voicemail.
Due to the COVID-19/Coronavirus Pandemic and our concern for its impact on our community, we have amended some of our qualifying requirements. Please review below to determine if you might be eligible to become a Volunteers in Medicine (VIM) patient:
Are you an adult between the ages of 18-64? (We do not see children. Reach out to KidCare for health insurance coverage for children.)
Do you or your spouse/partner work at least 20 hours per week?
Were you or your spouse/partner working at least 20 hours per week in January and February 2020 prior to COVID-19/Coronavirus?
You do not have health insurance?
Your household gross (before tax) income falls in the range below or
Your household gross (before tax) income fell in the range below for the months of January and February prior to COVID-19/Coronavirus:

To apply to become a patient at our clinic please review the above qualifying requirements and send an email to email@example.com with the following information:
- First and last name
- Phone number
- Email address
- Date of Birth
- What brings you to Volunteers in Medicine?
- Number of people in family/household
- Number of hours worked per week
- Attach a copy of your last two months’ worth of pay stubs to the email
– If you are currently not working, but worked at least 20 hours per week in January and February of 2020, and earned income within the guidelines above, you may send a copy of your January and February pay stubs to apply.
