Your health is so important to us. We are a free clinic for the working uninsured offering primary care and 16 specialties. We focus on the health of the whole person and do it with the help of 250 volunteers a month. We serve over 1,000 of your neighbors a year and we want to be your primary care provider. Volunteers in Medicine | Patient Qualifying Guidelines
Please review below to determine if you might be eligible to become a Volunteers in Medicine (VIM) patient:
Are you an adult between the ages of 18-64? (We do not see children. Reach out to KidCare for health insurance coverage for children. If you think you may qualify for health insurance you can go to the Get Covered Jax! website. )
Do you or your spouse/partner work at least 20 hours per week?
You do NOT have health insurance?
Your household gross (before tax) income falls in the range below.

To qualify to become a patient at our South San Marco or West Jacksonville Clinic, please fill out the form below.
If you need additional assistance with the form below, please email Qualify@vimjax.org and provide your full name, the clinic to which you would like to be referred (South San Marco or West Jacksonville), and 6 weeks of paystubs. (If self employed, please include 3 months of bank statements.)
